A Clinical Review of Upper Gastrointestinal Bleeding

Abstract
Three hundred and eighty six upper gastrointestinal bleeding cases, admitted in Kyungpook National University Hospital during the past 10 years from January 1964 to December 1973 were revs ewe and statistic study on some clinical aspects were one. The results obtained were as follows.
1) The causative diseases producing upper gastrointestinal bleeding were in the order of peptic ulcer (41.4%), esophageal varices (24.9%), gastritis (13.7%), stomach cancer(7. 0%), esophageal cancer (1.6%) and Mallory-Weiss syndrome (0.3%). The incidence of upper gastrointestinal bleeding of unknown origin was 11.
2) The sex incidence of upper gastrointestinal bleeding revealed a greater prevalence in male with a ratio of 4.6 to I and the peak incidence was in the 5th and 6th decade.
3) The annual incidence of upper gastrointestinal bleeding from 1964 to 1973 revealed rather increasing tendency. The monthly distribution revealed the lowest incidence in January. In peptic ulcer bleeding the seasonal variation was not obtained.
4) Hematemesis only was noted in 132 cases (34.2 %) and melena only in 90 cases (23.3%) while hematemesis and melena were seen in 164 cases (42.5%). And hematemesis was most frequently (86.5%) in cases of bleeding from esophageal varices.
5) Among 386 cases, 197 cases (51. 0%) belonged to massive bleeding group by Cammock¢¥s criteria.
6) Among 386 cases, 305 cases were treated conservatively and 81 cases received operation. The over-all mortality rate was 12.4% (48 cases) and the mortality rate for patients treated conservatively was 14. 1% (43 cases), while for those surgically treated it was 6.2% (5 cases). Among 48 cases of death, the causes were mainly due to uncontrollable bleeding (18 cases), hepatic failure (13 cases) and renal shut-down related to sustained shock (10 cases) in this order.
7) The causative agents of hemorrhagic gastritis included alcohol, analgesics(mostly salicylates), herb drug and steroid in this order.
